Position Overview

We are seeking a dedicated and skilled Physical Therapist (PT) to join our team at Reliant Orthopedic and Sports in Union, New Jersey. As a full-time PT, you will play a critical role in delivering personalized physical therapy services and enhancing patient outcomes in a home care setting. Your primary responsibility will be to develop and implement treatment plans, rehabilitating clients to achieve their highest level of function, while adhering to the physician's prescriptions and the clients' individual needs.

Work Environment

This position requires working onsite at clients' homes across Union, New Jersey. You will engage directly with patients in their personal environments, offering a unique insight into their daily lives and challenges. This allows for tailored treatment interventions aimed at maximizing functional independence. A typical schedule for this role will be Monday to Friday, providing a balanced work-life environment while dedicating your efforts to enhancing patient care.

Key Responsibilities

• Direct physical therapy treatment based on individual patient care plans to prevent disability, alleviate pain, enhance muscle function, and promote wellness.
• Oversee and mentor physical therapy assistants (PTAs) to ensure effective execution of treatment plans in accordance with regulatory requirements.
• Conduct assessments of client's functional capabilities through diagnostic tests and exercises to establish a baseline for treatment.
• Evaluate clients' home environments for rehabilitation opportunities and provide tailored recommendations to improve their independence.
• Administer therapeutic exercises, traction, and massage, while also instructing clients in non-manual exercises to foster functional activities.
• Assess and adapt prosthetic and orthotic devices based on individual needs and document necessary modifications for the physician.
• Collaborate in the development, review, and updating of comprehensive care plans, ensuring alignment with patient goals.
• Observe, document, and report any changes in clients' conditions to the healthcare team, facilitating timely adjustments to their care plans.
• Participate in interdisciplinary team meetings, discussing client treatment strategies and outcomes with other healthcare professionals.
• Instruct clients and their families on the usage of physical therapy equipment, as well as educating them about their overall therapy program.
• Complete clinical documentation, including initial evaluations, care plans, progress notes, and discharge summaries, ensuring adherence to compliance standards.
• Remain accessible to PTAs during client treatment sessions to provide support and guidance as needed.

Patient Population / Client Focus

As a Physical Therapist at Reliant Orthopedic and Sports, you will work with a diverse patient population, including individuals recovering from surgery, managing chronic pain, or seeking rehabilitation post-injury. Your approach will involve tailoring interventions to meet the unique challenges faced by each client in their home environment, fostering their ability to navigate daily activities effectively.

Qualifications

• Graduation from an accredited Doctorate in Physical Therapy (DPT) program.
• A valid and active Physical Therapist license in the state of New Jersey with no history of sanctions or board proceedings.
• Demonstrated expertise in manual therapy techniques and a robust ability to design and implement evidence-based exercise programs.
• Proficient in Electronic Medical Records (EMR) software, particularly WebPT, for accurate and timely documentation.
• Strong understanding of current practices and advancements in physical therapy supported by research.

Preferred Experience

• Experience in home health care or similar settings is preferred but not mandatory.
• Proven track record of successfully leading treatment plans and enhancing patient care in a multidisciplinary team environment.

Compensation and Benefits

We offer a competitive salary ranging from USD $100,000 to $120,000 annually, complemented by a robust benefits package.
