This position will allow you to work remotely, providing you with flexibility while maintaining a collaborative atmosphere with your colleagues. Although the majority of your work will be done from home, you will regularly engage in clinical case conferences and team meetings through virtual platforms, ensuring constant communication and support from your fellow healthcare professionals.

Key Responsibilities\n

• Conduct comprehensive intake and psychosocial assessments to determine the needs and challenges faced by patients and their families.

• Develop, present, and advocate for individualized treatment plans in collaboration with the Clinical Director of Recovery Programs (CDRP) and interdisciplinary care teams, ensuring they are designed to meet patient-specific goals and objectives.

• Participate actively in clinical case conferences, contributing insights and recommendations aimed at improving patient outcomes and care strategies.
\n
• Lead educational lectures focusing on chemical dependency and its implications for patients, families, and significant others, fostering understanding and engagement.
\n
• Offer individual and group counseling services tailored to address chemical dependency challenges, employing evidence-based practices and methodologies.
\n
• Provide therapy sessions for adolescents, family members, and multi-family groups as needed, ensuring a broad-based approach to healing and recovery.

• Coordinate comprehensive case management services, linking families to vital community resources and support networks.
\n

Patient Population / Client Focus\n

Your primary focus will be on patients in hospice care along with their families—individuals who are often experiencing complex emotional and psychological challenges due to serious health conditions. This role requires a deep understanding of grief, loss, and mental health issues associated with end-of-life care, as well as the ability to provide compassionate support to foster resilience and coping strategies.

Qualifications\n

• A master's degree in social work (MSW) or a related field is mandatory.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Oregon is essential.

• A minimum of two years of relevant postgraduate experience in supervised addiction medicine therapy is required, with substantial focus on the past five years of your career.

Compensation and Benefits\n
