Work Environment

As part of an engaging and supportive team, you will work onsite as a mobile therapist, providing care primarily in patients' homes across Henrico and the surrounding areas. Our clinic emphasizes collaboration among healthcare professionals, allowing for a multidisciplinary approach to patient care. Key Responsibilities

• Conduct thorough evaluations of patients, including reviewing medical histories and performing specific assessments to create personalized treatment plans.

• Effectively communicate evaluation results and treatment plans to patients and their families to ensure comprehensive understanding and involvement in goal setting.

• Implement evidence-based treatment protocols tailored to individual patient needs, monitoring progress and adapting care plans as necessary.

• Collaborate with other healthcare providers to ensure a holistic approach to patient care, documenting any changes in patient condition that warrant adjustments to the treatment plan.

• Oversee discharge planning and follow-up care, ensuring that all documentation is accurate, timely, and reflective of the patient's ongoing care requirements.

• Demonstrate excellent time management by maintaining an organized schedule and upholding productivity expectations within the home care setting.

• Participate in community outreach initiatives and marketing activities to promote access to services and expand community awareness of our offerings.

• Engage in departmental goal-setting activities and adhere to accreditation standards while seeking opportunities for process improvement.

• Pursue continuous professional development through additional training and educational opportunities to enhance skills and competency.

Patient Population / Client Focus

Our patient population includes individuals of all ages who require physical therapy services due to a variety of conditions, including post-operative rehabilitation, chronic pain management, and mobility challenges. Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.

• Current state licensure as required in Virginia.

• American Heart Association's Basic Life Support (BLS) certification for Healthcare Providers is mandatory.

• While prior work experience is not required, any practical experience in a clinical setting or home care environment is a plus.
