Key Responsibilities\n

• Act as a liaison between clients, families, and outpatient providers, ensuring a continuum of care that includes frequent communication and collaboration.

• Engage with insurance companies for pre-certification and utilization management, simplifying the process for clients and their families.

• Develop and participate in comprehensive discharge and aftercare planning, tailoring each plan to fit the individual needs of clients to support successful transitions.

• Provide therapeutic meal support in group settings while modeling a healthy relationship with food, integrating nutritional perspectives into overall mental health improvement.

• Deliver empathetic, affirming care to clients, acknowledging and honoring their unique circumstances and needs based on intersecting identities, including cultural and life experiences.

Patient Population / Client Focus\n

In this role, you will primarily work with clients seeking detoxification and those navigating the complexities of substance use disorders. Your approach will be holistic, addressing not only the immediate therapeutic needs but also long-term wellness and recovery strategies. Compensation and Benefits\n
