Key Responsibilities\n

• Conduct thorough psychosocial assessments, evaluating patients' mental health status, strengths, and needs to inform individualized care plans.

• Provide evidence-based therapeutic interventions, utilizing modalities such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), addressing the unique emotional and psychological challenges faced by patients in hospice care.

• Collaborate with an interdisciplinary team of psychiatrists, nurses, and other healthcare professionals to ensure comprehensive and coordinated patient care throughout the hospice journey.

• Engage and educate patients and their families, helping them understand diagnoses, treatment options, and medication management, while facilitating informed decision-making.

• Facilitate individual, group, and family therapy sessions, employing therapeutic techniques designed specifically for those dealing with end-of-life issues.

• Develop and implement holistic treatment plans in collaboration with the treatment team, ensuring that patient goals and preferences are central to the care process.

• Navigate patients through the complex mental health system, advocating for their rights, needs, and preferences to optimize their care experience.

• Lead therapeutic groups and psychoeducation sessions, in addition to participating in discharge planning meetings to ensure a smooth transition in care.

Qualifications\n

Compensation and Benefits\n
