Key Responsibilities\n

• Design and facilitate educational and therapeutic groups that address the specific psychosocial needs of patients, documenting responses and tracking progress.

• Participate actively in treatment-planning meetings, offering insightful feedback on patient progress and potential barriers while meticulously updating treatment plans.

• Identify and address resistance and conflict among clients, whether they are patients, family members, or part of the healthcare team, employing therapeutic techniques to reduce tension and promote effective coping strategies.

• Maintain comprehensive and accurate patient records that detail assessments, treatment plans, interventions, and case closures in alignment with ethical social work standards.

• Develop personalized treatment plans that highlight patient strengths, identify deficits, and address high-risk psychosocial needs to ensure tailored care delivery.

You will work primarily with patients in hospice care, a sensitive and critical stage where emotional and psychological support are essential. Qualifications\n

Compensation and Benefits\n

We offer a competitive annual salary range of USD 120,000 to 130,000 , based on your experience and caseload management.
