Key Responsibilities\n

• Conduct clinical assessments and evaluations for patients upon admission to establish their unique needs and develop personalized treatment plans.

• Begin discharge planning on the day of patient admission, ensuring that all aspects of the transition process are addressed from the start.
\n
• Facilitate discharge sessions with patients and their families, aiding their understanding of coping strategies, available resources, and ensuring that transition plans are clearly communicated.

• Confirm follow-up appointments with patients and their loved ones to ensure continuity of care upon discharge.
\n
• Actively communicate with referral sources, outpatient providers, and the clinical team to maintain ongoing communication regarding patient status at key points: admission, midpoint, and discharge.
\n
• Assist in the crisis management process during therapy sessions and throughout the clinical setting, supporting the implementation of de-escalation techniques as necessary.
\n
• Empower milieu (environmental) staff by providing guidance on effective engagement with patients and their families, ensuring a unified approach to care.
\n
• Participate in outreach initiatives and programs that enhance community awareness and engagement with hospice services.
\n
• Regularly assess patients for risk factors such as suicidality and potential harm to self or others, taking proactive steps to ensure their safety.
\n
• Provide additional support to clinical team members during their absence, ensuring uninterrupted care and service delivery.
\n
• Collaborate with the Utilization Review and Business Departments to ensure timely authorizations and payment processes.

Qualifications\n

• A Master's degree in Social Work, Counseling, or Psychology is required.

• Must hold an active and unrestricted license as a Licensed Clinical Social Worker (LCSW).

• Strong organizational skills allowing for timely documentation of services rendered, ensuring compliance with all regulatory requirements.

Preferred Experience\n

• A minimum of two years of direct social work experience in a clinical or hospice setting is preferred.

• Experience conducting discharge planning and facilitating psychoeducational groups is highly desirable.

• Familiarity with mental health documentation, including initial assessments, treatment planning (SMART goals), and clinical case notes.

• Background in crisis intervention techniques and managing complex patient situations.
