This remote role allows you the flexibility to perform your duties from a location of your choosing while remaining connected with our dynamic team of professionals. You will interact with an interdisciplinary team through telehealth sessions, making effective use of digital tools to document services and care plans electronically, enhancing collaborative practice and patient outcomes.

Key Responsibilities\n

• Conduct thorough, age-appropriate bio-psychosocial assessments to determine the mental health, substance abuse, and overall medical needs of patients within the primary care and hospice setting.

• Provide a range of behavioral health services, including individual and family counseling, intakes, diagnostic evaluations, treatment planning, crisis intervention, and referrals to substance use treatment resources.
\n
• Engage with patients and their families to plan and facilitate effective counseling sessions, adapting techniques and approaches based on each client's unique needs and circumstances.
\n
• Maintain comprehensive electronic records, documenting mental health evaluations, progress notes, and suicide risk assessments, while complying with confidentiality regulations and best practices.
\n
• Collaborate with the Quality Improvement team to develop processes for data collection that enhance service delivery and patient care outcomes.
\n
• Act as a liaison to local resources and community agencies to provide essential support services, including navigating medication assistance programs and other community resources.

Patient Population / Client Focus\n

Your role focuses primarily on providing palliative care to individuals facing the challenges of terminal illnesses and their families. You will use your skills to foster emotional resilience, facilitate coping strategies, and enhance the quality of life for patients in their final stages. You will encounter diverse populations, necessitating an understanding of various cultural perspectives on death, dying, and the grieving process.

Qualifications\n

• A valid and current LCSW license in the state of California is required.

• A Master’s degree in Social Work from an accredited program.
\n
• Minimum of three (3) years of post-licensure experience in clinical social work, with a focus on mental health and substance use treatments.

• Strong knowledge of counseling techniques, crisis intervention strategies, and the ability to exercise independent judgment in clinical decision-making.
\n
• Demonstrated experience in group facilitation and the development of programmatic plans.

Preferred Experience\n

• Prior experience in a hospice or palliative care setting is highly desirable.

• Experience working with diverse populations and a strong understanding of cultural competence in healthcare.

Compensation and Benefits\n

The Keck Health Care Center offers a competitive salary range of USD 115,000 – 130,000 annually, commensurate with qualifications and experience. Additionally, our comprehensive benefits package includes:
