Key Responsibilities\n

In your role as an LCSW, you will be charged with several essential functions:

• Conduct thorough evaluations and assessments to identify the needs of children and families.

• Develop and implement Individualized Family Service Plans (IFSPs) tailored to the unique situations of each family.

• Monitor and evaluate outcomes, using evidence-based practices and a coaching model to enhance service delivery.

• Establish trusting relationships with children, families, and team members to create a nurturing learning environment.

• Provide families with vital information, skills, and support to enhance their child's developmental progress, including guidance on developmental milestones.

Patient Population / Client Focus\n

This role specifically serves a vulnerable population — babies and toddlers with developmental delays or disabilities. Qualifications\n

The ideal candidate for this position should possess the following qualifications:
