Work Environment

As a Physical Therapist at Swift Health, you will work onsite in a home care setting. This unique work environment allows you to develop personalized treatment plans in the comfort of patients' homes, promoting relaxation and comfort during therapy sessions. Key Responsibilities

• Conduct comprehensive evaluations of patients, reviewing pertinent medical histories and performing specific assessments to identify their needs and tailor individualized treatment plans.
• Clearly explain evaluation findings and proposed treatment plans to both patients and their families, ensuring they are engaged in the goal-setting process.
• Demonstrate expertise in implementing treatment protocols based on evaluation findings, staying adaptable to the evolving healthcare landscape and individual patient presentations.
• Collaborate with multidisciplinary teams, recognizing when to adjust care plans or seek additional consultations based on changes in patients' conditions.
• Maintain meticulous documentation of patient progress, including legible daily encounter notes and discharge evaluations, to ensure continuity of care.
• Manage time effectively to meet productivity goals, maintain an organized schedule, and actively participate in outreach initiatives to enhance community health awareness.
• Contribute to departmental goal setting and adhere to compliance regulations, ensuring the highest standards of practice are maintained.
• Engage in continuous professional development through ongoing education opportunities, while also mentoring and supporting new practitioners and students.
• Promote exceptional customer experiences, fostering strong relationships with patients to enhance access to care and overall satisfaction.

Qualifications

• A Bachelor's Degree in Physical Therapy is required.
• Must possess a valid state license to practice as a Physical Therapist in Florida.
• Current American Heart Association BLS for Healthcare Providers CPR certification is necessary.

Preferred Experience

Although a minimum of two years of experience in a related field is preferred, we value a strong foundation of knowledge and a passion for patient care. Compensation and Benefits
