Work Environment

This role involves onsite interactions with clients in their homes, allowing you to evaluate their conditions in real-world settings. The home care environment requires adaptability and creativity, as you'll be tasked with making the most of each client's living situation to facilitate recovery. You'll be working closely with patients and their families, often requiring significant collaboration to enhance recovery efforts.

Key Responsibilities

• Develop and implement individualized physical therapy treatment plans that address each client's specific needs, focusing on relieving pain, preventing disability, and enhancing muscle function.
• Direct and oversee the activities of physical therapy assistants to ensure the treatment plan is being executed accurately and effectively.
• Conduct thorough evaluations of clients' functional abilities using diagnostic assessments to tailor therapy interventions accordingly.
• Assess and modify the home environment to optimize conditions for rehabilitation and client independence.
• Provide hands-on therapy services in accordance with physician prescriptions and clinical standards.
• Administer therapeutic exercises, assist clients in their rehabilitation routines, and ensure proper use of any needed equipment.
• Monitor patients' progress, document changes in their condition, and communicate effectively with physicians and interdisciplinary team members regarding care plan adjustments.
• Create and maintain comprehensive clinical records, including initial evaluations, progress notes, and discharge summaries.
• Host educational sessions for clients and families, guiding them through therapeutic processes and device usage.
• Participate actively in case conferences, contributing to a collaborative team approach focused on holistic patient outcomes.

Patient Population / Client Focus

You will be responsible for a diverse range of clients, including individuals recovering from surgeries, those managing chronic conditions, and elderly patients requiring rehabilitative services. Each client's unique circumstances will require you to employ a tailored approach to care, ensuring that you provide empathetic, holistic support throughout their rehabilitation journey.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• Active and valid Physical Therapy license in Florida.
• Current CPR certification and valid driver's license with automobile insurance.
• Strong interpersonal and communication skills necessary for patient and family engagement.

Preferred Experience

Compensation and Benefits
