Work Environment

This position is centered in a home care setting, where you will work in patients' residences to provide personalized therapy tailored to their unique circumstances. This onsite role allows you to build meaningful relationships with patients and their families in a comfortable, familiar environment. As part of our team, you will have the autonomy to determine the best treatment delivery methods while collaborating with an interdisciplinary team, including physicians and other healthcare professionals.

Key Responsibilities

• Conduct comprehensive evaluations of patients to assess their physical abilities, limitations, and needs.
• Design, implement, and adjust individualized treatment plans that address specific functional limitations, ensuring the best outcome for each patient.
• Provide education and training to patients and their caregivers on therapeutic exercises, mobility techniques, and self-care strategies to promote independence.
• Maintain accurate and timely documentation of patient records, tracking progress, treatment adjustments, and adherence to legal and regulatory standards.
• Engage in collaborative discussions with the interdisciplinary treatment team to ensure integrated care and continuity for patients.
• Participate in community outreach programs that promote awareness and education about physical therapy and preventive health.
• Stay updated with the latest developments in the field of physical therapy through continuous education and professional development opportunities.

Patient Population / Client Focus

The ideal candidate will work with a diverse patient population, including elderly individuals, post-surgical patients, and those recovering from injuries or strokes. You will provide tailored care to address various conditions, such as orthopedic injuries, neurological disorders, and chronic pain, ensuring that treatment is sensitive to each patient's cultural background and individual needs.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program is required.
• Valid state license to practice physical therapy in Florida and current Basic Life Support (BLS) certification.
• Strong verbal and written communication skills to effectively interact with patients, their families, and team members.
• Proficiency in electronic health record (EHR) systems and basic computer applications.

Preferred Experience

• A minimum of 2 years of experience as a Physical Therapist, particularly in home care or outpatient settings.

Compensation and Benefits
