Work Environment

This full-time position operates onsite, specifically within a home care setting. As a Physical Therapist, you will travel to clients' residences in Fort Lee and surrounding areas, offering flexibility and variety in your daily work routine. Your schedule will be Monday to Friday from 9 AM to 5 PM, allowing for an engaging daytime work life dedicated to patient care. You will collaborate with nursing staff and other health professionals to provide integrated support for our patients.

Key Responsibilities

• Conduct thorough physical therapy evaluations for each client, assessing their individual mobility challenges and rehabilitation needs.
• Develop, implement, and modify personalized treatment plans based on the evaluation findings, ensuring to keep them aligned with the patients' goals and progress.
• Deliver one-on-one therapeutic sessions focusing on enhancing strength, balance, mobility, and pain management strategies tailored to each client's situation.
• Monitor and document patient progress meticulously, using evidence-based practices to adjust treatment plans as necessary for optimal outcomes.
• Work closely with an interdisciplinary team, including nurses and occupational therapists, to coordinate care that is holistic and patient-centered.
• Provide education to clients, their families, and caregivers on the effective management of their conditions, proper body mechanics, and fall prevention techniques.
• Maintain thorough and accurate documentation of patient care activities and outcomes according to company policies and regulatory standards.

Patient Population / Client Focus

The primary focus of this position includes a diverse array of adult patients, predominantly aged 40 to 65 and older, requiring rehabilitation due to various conditions including post-surgery recovery, chronic pain, and cardiovascular issues. Qualifications

• Current and active Physical Therapist (PT) license in New Jersey.

• Ability to work independently, demonstrating initiative and confidence in providing treatment.

Preferred Experience

Compensation and Benefits
