Licensed Physical Therapist - Senior Rehabilitation Center

Work Environment

This position will be based onsite at our Rehabilitation Center , where you will directly interact with residents in a calm residential setting that promotes healing and well-being. Key Responsibilities

• Conduct comprehensive physical therapy evaluations for new and existing residents to assess their functional capabilities and identify rehabilitation needs.
• Design and implement personalized, goal-oriented treatment plans that account for each resident's unique health conditions and desired outcomes.
• Provide individualized therapy sessions, which may include exercises for strength, balance training, and mobility enhancement, in residents' apartments or in dedicated therapy spaces.
• Continuously monitor residents' progress throughout treatment, making necessary adjustments to their therapy plans based on ongoing evaluations and outcomes.
• Maintain accurate and detailed documentation of patient care activities using our Electronic Medical Records (EMR) system, ensuring compliance with regulatory and organizational standards.
• Collaborate closely with nursing, wellness staff, and caregivers to coordinate comprehensive care and promote successful implementation of fall prevention programs and safety protocols.
• Educate residents and their families on effective mobility strategies, proper use of assistive devices, and home-based exercise regimens that support ongoing health and independence.

Patient Population / Client Focus

This position focuses primarily on engaging with older adults residing in our Assisted Living Facility. As a Physical Therapist, you will play a pivotal role in enhancing their quality of life through tailored rehabilitation plans aimed at increasing strength, flexibility, and overall functional independence. Your passion for working with seniors will be essential in supporting and encouraging residents to achieve their mobility-related goals.

Qualifications

• Active and unrestricted New Jersey Physical Therapist License .
• A degree from an accredited Physical Therapy program, such as DPT or MPT.
• Current CPR/BLS certification to ensure the safety of our residents.
• Exceptional interpersonal and communication skills, demonstrating the ability to connect with diverse individuals.
• A compassionate attitude and a commitment to enhancing the lives of older adults.

Preferred Experience

• At least two years of experience in a clinical setting, preferably working with geriatric populations.
• Demonstrated proficiency in using EMR systems and adapting treatment protocols based on individual progress.
• Experience in rehabilitative techniques specifically tailored to residents with orthopedic and neurological conditions.

Compensation and Benefits

This full-time role offers a competitive salary ranging from $100,000 to $120,000 , commensurate with experience and qualifications. In addition to a rewarding salary, we provide a comprehensive benefits package that includes:
