Key Responsibilities\n

• Conducting thorough psychosocial assessments of patients and their families to understand their emotional, social, and environmental factors related to their diagnoses and life situations.

• Formulating, developing, and executing individualized treatment plans based on assessments, integrating a variety of therapeutic modalities to provide comprehensive support.

• Collaborating consistently with a multidisciplinary healthcare team, ensuring that patient and family involvement is central to the planning and implementation process.

• Facilitating effective discharge planning by coordinating with consulting physicians, insurance providers, and other healthcare professionals to ensure smooth transitions and continuity of care.

• Providing education and counseling to patients and families regarding adjustment issues related to the patient's illness and potential challenges post-treatment.
\n
• Utilizing appropriate crisis intervention techniques and therapies, offering both short-term solutions and ongoing individual, group, and family counseling sessions.

• Acting as a resource for other staff members by providing expert consultation on complex social work issues.
\n
• Offering education to healthcare professionals to foster a broader understanding of psychosocial factors affecting patient care.

• Evaluating oncology patients for signs of depression and anxiety, delivering short-term counseling as needed.
\n
• Participating in an on-call rotation to address after-hour patient concerns and crises.
\n
• If clinically indicated, executing independent involuntary examinations under the Baker Act, ensuring proper transfer protocols to receiving facilities.

• Presenting educational workshops and conferences on psychosocial oncology practices, maintaining a presence as an expert in the field.

• Contributing to community services through education and research initiatives, supporting resources for cancer survivors.

Patient Population / Client Focus\n

Your primary focus will be individuals in acute stages of addiction and mental health crises, as well as patients managing chronic illnesses such as cancer. You will engage with patients of diverse backgrounds, providing empathetic care and tailored interventions that honor their unique experiences and circumstances.

Qualifications\n

• A Bachelor’s Degree in Social Work (BSW) from an accredited institution.

• Current and valid Licensed Clinical Social Worker (LCSW) certification.

• Strong interpersonal and communication skills, with the ability to build rapport with patients and their families.

• Demonstrated ability to work effectively in a multidisciplinary team environment.

• Knowledge of mental health and addiction treatment modalities.

Preferred Experience\n

• Prior experience in a hospital or detox center setting is a plus.

Compensation and Benefits\n

Entryway Family & Psychiatric Services offers a competitive salary range of USD 70,000 – 100,000 per year, commensurate with experience.
