Care Manager: Frontline Addiction Recovery & Outreach
Crossroads
Crossroads Treatment Centers is seeking a Care Manager to support patients with opioid use disorder through coordinated care and resource connection. You will work with a team to deliver evidence-based services and address gaps in care across our outpatient clinics. The role requires a High School Diploma or GED, experience in healthcare or addiction medicine, and ability to build individualized care plans.
