Key Responsibilities

• Conduct thorough evaluation services, including the review of patients' medical histories and the execution of evidence-based assessments to establish individualized treatment plans.
• Communicate evaluation findings and crafted treatment strategies clearly to patients and their families, actively involving them in setting rehabilitation goals and expectations.
• Implement and demonstrate treatment protocols based on evaluation outcomes and the evolving needs of each patient, utilizing the latest modalities and techniques.
• Work collaboratively with other healthcare providers to monitor patient progress, recognizing and documenting changes in clinical status, which may necessitate adjustments to care plans.
• Oversee comprehensive discharge planning and follow-up care, documenting patient encounters with precision. This includes completing initial evaluations, daily notes, interim summaries, and discharge evaluations.
• Adhere strictly to productivity expectations while managing scheduling efficiently. Participate actively in marketing initiatives to promote our clinic and engage with local community events.
• Contribute to departmental goals by participating in ongoing quality improvement projects and complying with regulatory standards and best practices.
• Engage in continuing education pursuits to enhance your own expertise as well as mentor aspiring physical therapists through clinical instruction and hands-on training.

Patient Population / Client Focus

In this role, you will work with a diverse patient population that includes individuals recovering from surgery, sports injuries, chronic pain conditions, and age-related issues. Your practice will emphasize a holistic and tailored approach, ensuring that each treatment plan addresses the unique needs and goals of every patient, thereby enhancing their quality of life and functional independence.

Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited program.
• Valid licensure to practice physical therapy in New Jersey.
• Current American Heart Association BLS certification for Healthcare Providers.
• Dedication to patient-centered care and the ability to work collaboratively within a team environment.

Preferred Experience

• A minimum of 2 years of experience in an outpatient physical therapy setting is preferred, allowing for a smoother integration into our team and patient care protocols.

Compensation and Benefits
