Key Responsibilities\n

• Conduct thorough behavioral health assessments for all new patients to establish individualized care plans based on their unique needs.

• Manage ongoing treatment for patients by addressing their psychiatric, psychosocial, physical health, and behavioral health needs throughout their entire treatment journey.

• Facilitate both individual and group therapy sessions, utilizing evidence-based practices to empower patients on their path to recovery.

• Work collaboratively with an interdisciplinary care team, including primary care providers and external behavioral health facilities, to ensure a holistic approach to patient care.

Qualifications\n
