Key Responsibilities\n

• Conduct thorough clinical evaluations to develop tailored treatment plans, focusing on each client's unique needs.

• Engage actively with clients to create aftercare plans that promote successful transitions post-discharge.

• Document interactions and treatment plans according to organizational standards, maintaining accurate and up-to-date records.

• Participate in multidisciplinary case management meetings, contributing insights and strategies to enhance team coordination.

• Provide crisis intervention support to clients facing immediate emotional distress, ensuring their safety and stabilization.

• Facilitate both individual and group therapy sessions that empower clients, encourage self-exploration, and promote recovery.

• Advocate for client access to community resources such as food assistance, housing, childcare, and healthcare, enhancing their overall well-being.

Patient Population / Client Focus\n

You will work with a diverse clientele, including individuals with complex mental health needs, substance use disorders, and significant life challenges. Your role is pivotal in delivering culturally competent care that considers each client’s background, emphasizing understanding, empathy, and active participation in their healing processes.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution.

• Active and unrestricted California Clinical Social Worker License (LCSW).

• At least 2 years of relevant experience in a behavioral health setting, demonstrating your ability to engage with diverse clientele.

Compensation and Benefits\n
