Key Responsibilities

• Conduct thorough evaluations of patients' physical abilities, limitations, and conditions to determine appropriate treatment plans specific to home care or in a facility setting.
• Develop and implement individualized treatment strategies utilizing a variety of therapeutic techniques, including exercises, mobilization techniques, and patient education to alleviate pain and enhance mobility.
• Document all patient interactions meticulously, tracking progress and modifying treatment plans based on patient feedback and recovery milestones.
• Regularly reevaluate patients to assess progress and redefine goals to ensure continuous improvement and adapt treatment approaches as necessary.
• Educate patients and caregivers on the proper use of assistive devices and techniques that support self-care and independence.
• Participate in interdisciplinary team meetings to provide updates on patient progress and collaborate on collective care strategies.
• Stay current with emerging trends in physical therapy through ongoing professional development, workshops, and maintaining professional affiliations.

Patient Population / Client Focus

Qualifications

• Bachelor's Degree in Physical Therapy.
• Current and valid state license in Physical Therapy.
