Key Responsibilities\n

• Conduct comprehensive assessments to identify patients’ psychosocial needs and develop tailored treatment plans in collaboration with the team.

• Facilitate short-term counseling sessions, providing crisis intervention and case management services to support clients in distressed situations.
\n
• Assist clients in navigating entitlements, completing necessary forms for supplemental insurance, and accessing social support benefits to reduce reliance on emergency services.

• Support clients in securing prescription cost assistance through partnerships with pharmaceutical companies and community resources.
\n
• Act as an advocate for clients, engaging with community agencies and resources on their behalf to ensure they receive appropriate support.

• Maintain comprehensive and accurate patient records, ensuring documentation meets regulatory standards while keeping track of treatment progress.

• Coordinate transportation assistance for clients when required, promoting access to necessary services or appointments.
\n
• Participate in clinical and team meetings to discuss case progress, treatment strategies, and share best practices in mental health care.

Patient Population / Client Focus\n

Your practice will focus on a diverse patient population, including individuals and families facing various mental health challenges such as anxiety, depression, trauma, relationship issues, and adjustment disorders. Through telehealth, you will extend your reach, providing valuable mental health support to clients who may otherwise have limited access to traditional therapy settings.

Qualifications\n

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required.

• Applicants must hold one of the following independent licenses: Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

• A minimum of two (2) years of professional experience in a mental health counseling role is essential, demonstrating your capability to handle various client cases.

Preferred Experience\n

Compensation and Benefits\n
