This full-time position is remote, allowing for flexibility in your schedule while you contribute to the supportive environment of our Hospice Center. Key Responsibilities\n

• Conduct in-depth psychosocial assessments to evaluate individual mental health concerns, identifying both strengths and challenges.

• Deliver individual, family, and group therapy using evidence-based interventions tailored to meet the specific and cultural needs of clients.

• Develop and implement treatment plans that incorporate measurable goals, evidenced by ongoing evaluations and adjustments based on client feedback and progress.

• Collaborate with a range of professionals—including psychiatrists and medical staff—to ensure comprehensive care that addresses both mental and physical health needs.
\n
• Provide crisis intervention services, conducting risk assessments and creating safety plans to protect clients in distress.
\n
• Act as an advocate for clients’ rights, connecting them with essential community resources and offering case management support.
\n
• Participate actively in community outreach to raise awareness about mental health services, aiming to reduce stigma and increase accessibility within diverse communities.

Patient Population / Client Focus\n

The LCSW will work with a broad range of clients, including children, adolescents, adults, and families facing various mental health challenges. Qualifications\n

Compensation and Benefits\n
