Key Responsibilities\n

• Perform comprehensive client assessments to understand and document psychological, social, and emotional issues, ensuring the development of tailored treatment plans that align with individual client needs.

• Deliver high-quality therapy sessions, including individual, couples, and family therapy, tailored to clients grappling with emotional distress related to grief and loss.

• Collaborate closely with physicians, nurses, and other healthcare professionals to provide a seamless continuum of care, ensuring that all aspects of the client’s emotional and physical well-being are addressed.

• Maintain accurate clinical records and progress notes using our proprietary electronic health record (EHR) system, ensuring all documentation is up to date and compliant with regulatory standards.

• Participate in regular clinical supervision, team meetings, and professional development workshops to enhance your skills and stay current with best practices in social work.

• Uphold adherence to all ethical and legal standards pertinent to social work and client confidentiality across all interactions and documentation.

Patient Population / Client Focus\n

This position primarily works with clients involved in hospice care, including patients facing terminal illness and their families. Understanding the complexities of grief, loss, and adjustment is essential, as you will provide emotional support and therapeutic interventions aimed at fostering resilience and coping mechanisms during difficult times.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Florida.

• Minimum of three years of experience in a mental health or outpatient setting, with a strong emphasis on end-of-life care or palliative settings being a significant advantage.

Compensation and Benefits\n
