Restore Physical Therapy is actively seeking a dedicated and compassionate Physical Therapist to join our home care team in Brooklyn, New York. In this full-time position, you will focus on delivering exceptional therapeutic care at patients' homes, implementing personalized treatment plans and driving the rehabilitation journey for diverse patient populations.

Work Environment

This role is primarily onsite, focusing on home care settings where you will engage with patients in their own environments. By building rapport and trust in their homes, you will address individual challenges and foster an atmosphere of encouragement and recovery. We operate Monday through Friday from 8:00 am to 5:00 pm, with occasional flexibility to accommodate varying patient needs. This work environment allows you to be integral to the patient's recovery process while managing your schedule effectively.

Key Responsibilities

• Develop and implement individualized treatment plans for patients, focusing on improving mobility, strength, and overall functionality.
• Guide patients through therapeutic exercises, stretches, and activities tailored to promote physical well-being and alleviate pain.
• Educate patients and their families on body mechanics, exercise techniques, and home exercise programs to foster ongoing recovery.
• Administer therapeutic modalities such as electrical stimulation, ultrasound, and heat/cold therapies under the guidance of your licensed colleagues.
• Monitor and document patient progress meticulously, tracking improvements in range of motion, strength, and functional capacities.
• Prepare treatment spaces and ensure that all necessary equipment and supplies are readily available and organized for therapy sessions.
• Collaborate with an interdisciplinary team of healthcare professionals, including Occupational Therapists and Speech-Language Pathologists, to integrate care strategies and optimize patient outcomes.
• Provide emotional support and encouragement to patients, creating a motivating and safe environment throughout their rehabilitation journey.
• Adhere strictly to ethical and professional standards, maintaining patient confidentiality and respecting their rights at all times.

Patient Population / Client Focus

You will work with a diverse patient population, ranging from elderly individuals requiring geriatric rehabilitation to athletes needing sports injury recovery solutions. Our focus is on personalized care that respects the unique challenges and ambitions of each patient. Your role is crucial in fostering therapeutic relationships that empower patients to take an active role in their recovery.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Valid state license to practice as a Physical Therapist in New York.
• Strong interpersonal skills and a passion for helping others recover and achieve their physical therapy goals.
• Ability to work independently while demonstrating strong organizational and time management skills.
• Commitment to ongoing professional development and adherence to ethical standards in patient care.

Preferred Experience

• At least one year of experience in physical therapy, preferably in a home care or outpatient clinic setting.
• Experience working with various patient populations, including geriatrics, pediatrics, and those requiring sports rehabilitation.

Compensation and Benefits
