Work Environment

Located in the vibrant city of San Jose, our rehabilitation center offers a welcoming and modern environment tailored to enhance both patient and staff experiences. Working onsite, our physical therapists collaborate closely with physicians, nurses, and other healthcare professionals to create a cohesive treatment team. The work schedule for this position is Monday to Friday from 2 PM to 8 PM, providing a comprehensive approach to rehabilitation during evening hours to accommodate our patients' needs.

Key Responsibilities

• Provide high-quality, patient-centered care by managing the complete therapeutic process for an assigned group of patients.
• Conduct thorough patient assessments and actively engage in developing individualized care plans aimed at achieving specific rehabilitation goals.
• Deliver or supervise the majority of therapeutic interventions, ensuring continuity of care for patients throughout their recovery journey.
• Perform evaluations and treatment sessions in accordance with the established practice standards and ethical guidelines.
• Assess and evaluate patients' therapeutic, rehabilitative, and functional status, contributing to the development of a comprehensive plan of care in collaboration with the healthcare team.
• Provide hands-on therapeutic interventions at the rehabilitation center or at patients' residences, employing techniques that align with each patient's physician-prescribed plan of care.
• Educate patients, families, and caregivers on home exercises, therapeutic appliances, and prosthetic devices, ensuring understanding and compliance outside of therapy sessions.
• Prioritize patient caseloads effectively, determining daily staffing requirements and coordinating transitions of care.

Patient Population / Client Focus

As a Physical Therapist at Mobility Physical Rehabilitation Inc., you will work with a diverse patient population, addressing needs across various age groups from pediatrics to geriatrics. You will be instrumental in treating conditions related to post-operative recovery, sports rehabilitation, and chronic pain management, ensuring that each patient receives tailored care that focuses on restoring function and enhancing quality of life.

Qualifications

• Valid and unrestricted Physical Therapy license.
• Doctor of Physical Therapy (DPT) degree.
• Basic Cardiac Life Support (BCLS) certification, or willingness to obtain within 30 days of employment.
• No prior experience is required; new graduates are encouraged to apply.

Preferred Experience

• Previous clinical experience in an outpatient or rehabilitation setting is an asset.
• Familiarity with electronic medical record (EMR) systems to document patient progress and communicate effectively with the healthcare team.
• Strong interpersonal and communication skills to foster positive relationships with patients and colleagues.

Compensation and Benefits
