Work Environment

This position is onsite and primarily home care-focused, requiring travel to various patient locations within the San Jose area. Key Responsibilities

As a Physical Therapist at Carry On Physical Therapy, you will have diverse responsibilities, including:

• Conducting thorough evaluations for each patient to assess their physical condition, mobility levels, and specific rehabilitation needs.
• Developing customized treatment plans that are aligned with patients' personal goals and adjustments based on their progress.
• Implementing therapeutic exercises, manual therapy techniques, and functional training to facilitate recovery and enhance strength.
• Monitoring and documenting patient progress consistently to ensure that goals are being met and adjusting the treatment strategies as necessary.
• Educating patients and their families about safe techniques for carrying out exercises and activities to promote long-term health and prevent reinjury.

Patient Population / Client Focus

You will primarily work with a diverse range of patients, including those recovering from surgeries, injuries, and chronic conditions. Our emphasis is on orthopedics and musculoskeletal rehabilitation, with a focus on older adults who may require specialized care to maintain independence at home. Your role is integral in guiding them to achieve functional mobility and improving their overall wellness.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required.
• Must possess or be eligible to obtain a valid Physical Therapist license from the California Board of Physical Therapy.
• Demonstrable strong problem-solving skills and the ability to adapt treatment plans based on patient responses.
• Excellent communication and interpersonal skills to build rapport with patients and collaborate effectively with healthcare teams.

Preferred Experience

While a minimum of two years' experience in a clinical setting is required, applicants with additional credentials such as OCS, MTC, or similar certifications will be highly regarded. Experience in home health care or outpatient orthopedic settings will also be beneficial.

Compensation and Benefits
