Key Responsibilities

As a Physical Therapist, you will be responsible for the following:

• Evaluating and assessing patients referred by physicians to establish personalized treatment plans.
• Implementing a variety of therapeutic interventions tailored to each patient's specific needs and conditions.
• Providing in-home physical therapy services that incorporate state-of-the-art methodologies and practices.
• Collaborating with other healthcare professionals to enhance patient care and maximize outcomes through a team-oriented approach.
• Educating patients and their caregivers about treatment plans, exercises, and strategies for ongoing management of their conditions.
• Monitoring patient progress and adjusting treatment plans as necessary to ensure optimal recovery.

Patient Population / Client Focus

In this role, you will have the opportunity to work with a diverse range of patient populations, including but not limited to:

• Orthopedic patients recovering from surgeries or injuries.
• Pediatric patients with developmental or functional challenges.
• Neurologic patients requiring rehabilitation following strokes or brain injuries.
• Athletes recovering from sports injuries.

Qualifications

The ideal candidate must possess the following qualifications:

• A Bachelor's Degree in Physical Therapy from an APTA or CAPTE-accredited program; Doctorate in Physical Therapy is preferred.
• A current California State License to practice Physical Therapy.
• Basic Life Support Certification from the American Heart Association.
• Strong communication skills, both written and verbal.
• Proficiency in word processing and computer applications.
• An inherent ability to build meaningful relationships with patients, fostering trust and collaboration throughout the treatment process.

Preferred Experience

While not mandatory, the following experiences are highly valued:

• Previous work experience in home health or outpatient settings.

Compensation and Benefits
