Work Environment

The work environment is dynamic and engaging, centered around a collaborative approach to patient care. As a Home Health Physical Therapist, you will provide treatment in various home settings, adapting to each patient's unique living situation. Our therapy sessions are designed to effectively integrate clinical strategies into the home environment, making rehabilitation both accessible and practical for our patients. You will work Monday to Friday from 9 AM to 5 PM, ensuring a consistent schedule that supports work-life balance.

Key Responsibilities

• Deliver personalized, evidence-based orthopedic therapy in patients' homes throughout designated regions of California.
• Collaborate closely with in-house physicians, Occupational Therapists, Chiropractors, and outpatient Physical Therapists to develop integrated care plans.
• Conduct thorough assessments of patients' safety, mobility, and functionality in their home environments, tailoring intervention techniques accordingly.
• Provide education and ongoing support to patients and their families, covering essential topics such as exercise regimens, body mechanics, and adaptive strategies for daily living.
• Utilize secure and user-friendly Electronic Medical Record (EMR) tools for accurate documentation of sessions and patient progress.
• Engage in interdisciplinary case reviews and contribute to continuous clinical education and staff training initiatives.

Patient Population / Client Focus

This position focuses primarily on patients recovering from orthopedic injuries, surgeries, and chronic musculoskeletal conditions. You will work with a diverse patient population, including individuals of all ages who require specialized rehabilitation services tailored to their particular challenges and recovery goals. Your role is integral in helping patients regain their independence and improve their overall quality of life.

Qualifications

• Current Physical Therapist license in the State of California (or eligibility for licensure).
• Minimum of 2 years of experience in physical therapy, preferably in a home health or outpatient orthopedic setting.
• Exceptional interpersonal and organizational skills, coupled with the ability to work independently in a home care setting.
• Proficiency in managing a varied patient caseload while remaining flexible to adapt to unexpected circumstances and patient needs.

Compensation and Benefits
