Key Responsibilities\n

• Facilitate therapeutic sessions with clients to explore and address their mental health concerns effectively.

• Collaborate closely with psychiatrists and other prescribers to ensure an integrated approach to managing clients' medications alongside your therapeutic interventions.

• Monitor and evaluate treatment outcomes as part of a multidisciplinary team, participating in discussions and decision-making to enhance patient care.

• Take an active role in continuity of care and discharge planning, ensuring clients have a roadmap for ongoing support.

• Design customized treatment plans informed by each client's psychiatric diagnosis, presenting problems, and treatment needs, aimed at maximizing rehabilitation and recovery.

• Document all patient interactions and care activities accurately in the electronic healthcare record system, adhering to our organizational policies and best practices.

Qualifications\n

• A Master’s degree in Social Work, Counseling, Psychology, or a related field is required.

• Current and valid California state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
