As a remote position within a hospice care setting, this role allows you to collaborate with an interdisciplinary team of healthcare professionals from the comfort of your own home. You will maintain communication through virtual means, and occasionally engage in home visits when necessary. Key Responsibilities\n

Your day-to-day responsibilities will include:

• Conducting comprehensive evaluations and assessments of children enrolled in our hospice program, identifying their psychological, emotional, and social needs.

• Developing and implementing tailored care plans in collaboration with families and the interdisciplinary team, ensuring alignment with family goals and priorities.

• Monitoring and documenting progress towards Individualized Family Service Plan (IFSP) objectives, adjusting strategies as needed based on family feedback and child development markers.

• Providing emotional support and essential resources to families facing the challenges associated with caregiving in a hospice setting.

• Utilizing evidence-based practices to create a nurturing and supportive environment that fosters trust and communication.

• Adhering to all relevant federal and state regulations while ensuring compliance with organizational policies related to patient care and privacy.

Patient Population / Client Focus\n

Your primary focus will be on families with children who have life-limiting conditions or disabilities. You will provide specialized emotional and psychosocial support to these families, assisting them in navigating the complex healthcare environment while advocating for their child's quality of life. Qualifications\n

To be eligible for this role, candidates must possess:

• A Master's Degree or Doctorate in Social Work from an accredited institution.

• Active licensure as a Licensed Clinical Social Worker (LCSW) with the California Department of Public Health.
