Full-Time Physical Therapist for Home Care Services

Position Overview

The Physi Core Solutions is actively seeking a dedicated and skilled Physical Therapist to join our growing team in Pembroke Pines, Florida. In this full-time role, you will play a pivotal part in enhancing our patients' recovery journeys by providing exceptional onsite physical therapy services in the comfort of their homes. Work Environment

As a Physical Therapist at Physi Core Solutions, you will work onsite, primarily visiting patients' homes in Pembroke Pines and surrounding areas. This setting adds a unique dynamic to your role, as you will not only provide therapy but also assess the home environment and make necessary adaptations to support your patients' healing processes. You will collaborate with an interdisciplinary team, including Occupational Therapists and Speech-Language Pathologists, to ensure coordinated and effective care strategies.

Key Responsibilities

• Conduct comprehensive assessments and evaluations to craft individualized treatment plans that focus on enhancing mobility, strength, and overall functionality for each patient.
• Guide patients through therapeutic exercises, stretches, and other activities designed to alleviate pain and promote physical recovery.
• Educate patients and their families about appropriate body mechanics, effective exercise techniques, and the importance of adherence to home exercise programs for successful rehabilitation.
• Utilize therapeutic modalities, including electrical stimulation and ultrasound, to aid in treatment while ensuring safety and effectiveness under the physical therapist's supervision.
• Monitor progress meticulously, documenting key metrics such as range of motion and functional gains to evaluate the effectiveness of treatment plans.
• Prepare and maintain treatment areas and ensure the availability of necessary therapeutic equipment and supplies for efficient session delivery.
• Work collaboratively with other healthcare professionals to optimize patient care and outcomes through coordinated efforts.
• Foster emotional support and encouragement, addressing any anxieties or concerns the patients may have during their rehabilitation journeys.
• Uphold ethical standards and patient confidentiality, respecting the dignity and rights of each individual during therapy.

Patient Population / Client Focus

You will be working with a diverse range of patients, including individuals recovering from surgery, those coping with chronic pain, children with developmental challenges, and elderly patients recovering from strokes or other debilitating conditions. Your ability to connect with patients, understand their unique circumstances, and engage them in their own recovery will be crucial in your role.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current state licensure as a Physical Therapist in Florida.
• At least one year of experience in a physical therapy setting, preferably with a focus on home care.
• Strong interpersonal skills with the ability to build rapport with patients and their families.
• Excellent organizational skills to manage a caseload effectively and ensure timely follow-up with each patient.

Preferred Experience

• Experience with a diverse range of rehabilitation methodologies, including Manual Therapy techniques and therapeutic exercise.
• Familiarity with electronic health record (EHR) systems to facilitate documentation and reporting of patient progress.
• Background in patient education and the development of home exercise programs tailored to individual patient needs.

Compensation and Benefits
