Key Responsibilities\n

• Conduct thorough psychosocial assessments to inform treatment plans and establish initial discharge strategies to support patient recovery.

• Develop and manage initial treatment strategies based on assessment findings while coordinating additional services as required and adjusting plans as necessary.
