Job Opportunity: Licensed Physical TherapistsTrio Home Health (Jet Health, Inc.) is seeking a full time, part time or PRN / Per Diem Licensed Physical Therapists to join our team to providing PT services including assessment, treatment planning, implementation, and evaluation for our patients currently enrolled in our home health program in the Flower Mound, Lewisville, TX and surrounding areas.This position offers an exciting opportunity to make a meaningful impact on patients' lives while working in a dynamic and supportive environment.One on one patient care, work life balance, autonomy, great benefits and compensation.RequirementsTrio Home Health (Jet Health, Inc.) is dedicated to providing exceptional care to patients of all age groups in the comfort of their homes. We prioritize patient well-being and strive to deliver coordinated care that promotes healing and independence.Evaluate patients' physical and functional abilities to develop individualized care plans.Plan, implement, and modify physical therapy treatment programs based on patients' medical referrals and assessments.Provide hands-on therapy, exercises, and techniques to help patients improve mobility, relieve pain, and restore function.Educate patients and their families on therapeutic exercises, proper body mechanics, and injury prevention techniques.Collaborate with interdisciplinary healthcare teams to ensure coordinated and holistic patient care.Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment plans.Participate in ongoing professional development activities to stay abreast of current trends and best practices in physical therapy.Experience:Graduate of an accredited School of Physical Therapy.Current State license as a Physical Therapist, or a multi-state license issued by a state member of the National Physical Therapy Compact (PT Compact), if practicing in a state member of the PT Compact.Minimum of two years of experience as a Physical Therapist in a clinical care setting, with experience in Home Health, geriatrics, or medical-surgical preferred.
