Key Responsibilities\n

As a Licensed Clinical Social Worker, you will undertake a variety of responsibilities that are pivotal to patient care and support. Your daily tasks will include:

• Directing psychoeducational and process-oriented therapy groups tailored to meet the unique needs of our patients.

• Conducting individual therapy sessions focused on coping strategies, emotional regulation, and therapeutic support.

• Performing comprehensive clinical assessments to gauge patient needs upon admission, throughout care, and at discharge, ensuring a seamless treatment trajectory.

• Collaboratively communicating with the Utilization Review and Business Departments regarding authorization processes and payment issues for effective case management.
\n
• Initiating discharge planning on the day of patient admission, which includes setting goals for the transition to ensure continuity of care post-discharge.
\n
• Facilitating discharge sessions to reinforce coping skills and ensure patients and their families comprehend the transition plan and available resources.
\n
• Establishing and confirming discharge appointments with patients and their families to promote readiness and alleviated anxiety surrounding discharge.
\n
• Ensuring strong collaboration with referral sources and outpatient providers to guarantee a smooth continuum of care for patients.

Patient Population / Client Focus\n

In this role, you will primarily work with patients and their families who are facing terminal illnesses. This includes providing emotional support and counseling to both patients in hospice care and their loved ones, while addressing the complex psychosocial issues arising at the end stages of life. Your role is crucial in ensuring that patients can process their experiences, express their feelings, and gain support during this transformative time.

Qualifications\n

To qualify for this position, candidates must meet the following criteria:

• Master’s degree in Social Work from an accredited institution.

• Active license as a Clinical Social Worker (LCSW) in the state of California.

Compensation and Benefits\n
