Key Responsibilities\n

• Conduct detailed mental health assessments, including psychosocial evaluations, diagnostic interviews, and risk assessments to understand each patient's unique situation.

• Collaborate closely with patients and their families to create personalized treatment plans that cater to their specific mental health goals and accommodate cultural considerations.

• Provide evidence-based therapeutic interventions such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and Solution-Focused Brief Therapy (SFBT) in various formats: individual, couples, family, and group settings.

• Offer crisis intervention and management services for patients in acute distress, coordinating with other healthcare professionals to ensure seamless continuity of care.

• Maintain comprehensive and timely clinical documentation including progress notes, treatment plans, and discharge summaries according to our clinic's policies and regulatory requirements.

• Empower patients and their families through psychoeducation, helping them acquire the knowledge and skills necessary to manage their mental health challenges.

• Engage with a multidisciplinary team, including psychiatrists, psychologists, nurses, and case managers, to deliver a cohesive care experience for our patients.

• Perform regular outcome assessments to evaluate the success of treatment plans and suggest modifications as necessary.

• Stay abreast of the latest research and emerging trends in mental health, integrating this knowledge into your clinical practice to enhance patient outcomes.

Your work will primarily involve individuals and families facing mental health challenges within a hospice setting. Qualifications\n

• A Master's degree in Social Work (MSW) is required for this position.

Compensation and Benefits\n
