Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to identify the specific needs of children and their families.

• Participate actively in the Individualized Family Service Plan (IFSP) development process and its implementation, working closely with a multidisciplinary team.

• Monitor and evaluate outcomes in relation to child development, providing feedback and adjustments as necessary.
\n
• Develop strong, trusting relationships with families, utilizing effective communication strategies and evidence-based practices to build rapport and foster engagement.

• Provide direct support using the coaching model within natural learning environments, guiding families on strategies to enhance their child's development.
\n
• Consistently comply with all pertinent Federal and State regulations to ensure the safety and well-being of the families served.

• Utilize data-driven strategies when making recommendations based on family priorities and child status, facilitating a partnership approach to care.

Patient Population / Client Focus\n

This role primarily focuses on supporting children with disabilities or developmental delays from birth to three years old. You will engage with families to help infants and toddlers gain essential skills in physical, cognitive, social/emotional, and communication domains. Your efforts will significantly impact these children's early development and overall well-being.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work from an accredited institution is required.

• Current licensure as a Licensed Clinical Social Worker (LCSW) issued by the Department of Public Health.

• A valid driver's license is necessary for home visits when applicable.

• Strong organizational and time management skills to balance assessments, collaboration, and family support effectively.

• Demonstrated ability to customize approaches to accommodate various learning styles in both children and families.

Preferred Experience\n

Compensation and Benefits\n
