Work Environment\n

While you may have minimal on-site responsibilities at our Hospice Center, the majority of your engagement will occur through virtual interactions. You will collaborate with multidisciplinary teams including physicians, nurses, and other healthcare professionals, ensuring an integrated approach to patient care.

Key Responsibilities\n

• Conduct thorough assessments to evaluate patients’ mental health status, utilizing appropriate diagnostic tools, such as the Diagnostic and Statistical Manual of Mental Disorders (DSM), to inform individualized care plans.

• Implement and monitor evidence-based interventions to support patients in achieving their therapeutic goals while continually assessing their progress and adjusting care strategies as needed.

• Maintain meticulous documentation of treatment records, progress notes, and clinical reports in compliance with regulatory requirements and departmental protocols.

• Collaborate with patients, their families, and other healthcare providers to develop comprehensive and culturally sensitive treatment plans that address individualized patient needs.

• Provide education regarding mental health strategies and resources to patients and families, empowering them to engage actively in their care.

• Foster professional relationships with referral sources and community resources to facilitate comprehensive patient care.

• Mentor and support students and interns, sharing insights and best practices to contribute to the development of future healthcare professionals.

• Engage in peer reviews and clinical discussions under the guidance of your clinical supervisor to enhance team knowledge and cohesion.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Texas is required.

Compensation and Benefits\n

This full-time position offers a competitive salary ranging from $90,000 to $105,000 annually, depending on experience and qualifications.
