Key Responsibilities\n

• Conduct comprehensive assessments of patients’ emotional, behavioral, and social functioning, focusing on their mental health status and specific needs.

• Provide evidence-based psychotherapy interventions for a range of mental health conditions, including but not limited to anxiety, depression, grief, and post-traumatic stress disorder (PTSD).

• Develop and implement individualized treatment plans that are culturally competent and tailored to the unique needs of each patient.

• Collaborate effectively with interdisciplinary teams, including psychiatrists, nurses, and primary care providers, to coordinate comprehensive patient care.

• Monitor and document patient progress, thoroughly adjusting treatment plans based on clinical observation and patient feedback.

• Educate patients and their families about mental health issues, available treatment options, and community resources that can assist in care and support.

• Maintain high ethical standards, including ensuring patient confidentiality and obtaining informed consent for all treatments.

Patient Population / Client Focus\n

This position focuses on patients facing terminal diagnoses and their families, addressing the psychological aspects of end-of-life transitions. You will work closely with individuals experiencing grief and loss, requiring sensitivity and an understanding of the complex emotions involved in hospice care.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of 2-3 years of experience in providing psychotherapy and mental health services, preferably in a hospice or end-of-life care setting.

Preferred Experience\n

Experience in a hospice or palliative care environment is highly desirable, as it equips you with the insights needed to engage with patients and families under emotional stress. Experience with telehealth platforms is also a plus, considering the remote nature of the role.
