Key Responsibilities\n

• Conduct thorough psychosocial evaluations and assessments to identify the needs and challenges faced by clients and their families.

• Collaborate with the interdisciplinary team to develop and implement Individualized Family Service Plans (IFSP) that address the comprehensive needs of each client.

• Monitoring client progress and making necessary adjustments to care plans to enhance the effectiveness of interventions.

• Provide emotional support and counseling to families dealing with grief, loss, and complex feelings surrounding hospice care.

• Ensure compliance with all relevant Federal and State regulations, maintaining high standards of ethical practice and confidentiality.

Qualifications\n

• A Master's degree in Social Work (MSW) or a related field.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.
