Key Responsibilities\n

• Conduct comprehensive assessments of patients’ emotional and behavioral problems to develop tailored treatment plans.

• Provide counseling services, including crisis intervention, to help patients navigate their unique challenges, particularly in moments of acute distress.

• Maintain accurate and timely clinical documentation, including progress notes, in accordance with organizational policies and industry standards.

• Engage proactively with physicians and other healthcare professionals to communicate patient progress, needs, and any concerning developments.

• Collaborate with multidisciplinary teams to formulate and implement effective treatment plans and discharge planning.

• Establish and maintain productive relationships with community resource organizations to advocate for patient social needs and represent the organization's mission in various local committees.

Patient Population / Client Focus\n

This position specifically focuses on serving patients in a hospice care setting, who often face complex emotional and psychological issues related to terminal illnesses. You will work closely with patients and their families, providing support that respects their individual circumstances, cultural backgrounds, and specific end-of-life needs. Qualifications\n

• A minimum of three years of relevant professional experience in mental health counseling or social work.

• An active Master's degree in Social Work, Counseling, Marriage and Family Therapy, or Psychology from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW), Licensed Professional Clinical Counselor (LPCC), or Licensed Marriage and Family Therapist (LMFT) in the state of California.
