Key Responsibilities\n

• Utilize your clinical training to independently conduct psychosocial assessments and create effective treatment plans tailored to each patient's unique situation.

• Work collaboratively with patients to set realistic and achievable treatment goals, using evidence-based methodologies such as solution-focused therapy, motivational interviewing, and behavioral activation.

• Interface with a variety of healthcare professionals within our integrated system, including psychologists, primary care providers, and specialists, to ensure a comprehensive approach to patient care.

• Monitor patient progress consistently, employing both in-person and virtual check-ins to adjust treatment plans as necessary and document outcomes according to established standards.

• Assist in medication management, providing support to patients regarding adherence, potential side effects, and overall efficacy, with oversight from licensed medical professionals.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker in our hospice center, your typical clients will often include individuals facing end-of-life challenges, requiring sensitivity, empathy, and a profound understanding of grief and loss. You will work with them and their families, helping to navigate complex emotions and circumstances, ensuring they receive not only medical care but emotional and social support during critical times.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active and unencumbered license to practice as a Licensed Clinical Social Worker (LCSW) in Texas.

• A minimum of 1 year of experience functioning as an independently licensed therapist.
