Key Responsibilities\n

• Deliver individual and group counseling sessions tailored to each client’s unique treatment plan, including triage and crisis management when necessary.

• Conduct comprehensive assessments to evaluate mental health conditions and provide case management services, ensuring clients receive the proper referrals and resources for their recovery journey.

• Employ a variety of therapeutic approaches such as cognitive-behavioral therapy (CBT), motivational interviewing, and trauma-informed care to meet the diverse needs of clients in our detox facility.

• Collaborate with a multidisciplinary team during case consultations and meetings to create comprehensive care plans and improve coordination among various health providers.

• Plan and implement wellness outreach initiatives that educate clients and the community on mental health and addiction recovery.

Patient Population / Client Focus\n

As a Clinical Social Worker at Berks Community Health Center, you will be primarily focused on supporting individuals undergoing detox and those grappling with substance use disorders and associated mental health issues. Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

Compensation and Benefits\n
