The role is based remotely but is tied to a Community Health Clinic located in Port Saint Lucie, Florida. Key Responsibilities\n

• Conduct thorough evaluations and assessments of children to identify development needs.
\n
• Participate actively in the development and implementation of Individualized Family Service Plans (IFSP), creating tailored support strategies for each child and family.
\n
• Monitor and assess progress to ensure that outcomes align with the goals set in the IFSP, being an integral part of a multidisciplinary team.
\n
• Provide emotional and educational support to families, helping them learn and adopt effective practices that enhance their child’s development.
\n
• Utilize evidence-based practices and the Natural Learning Environment philosophy to foster an authentic learning atmosphere.
\n
• Facilitate ongoing communication with families to develop joint plans that reflect their priorities and concerns.
\n
• Ensure compliance with all relevant federal and state regulations, maintaining a safe and supportive environment for staff and clients.

Patient Population / Client Focus\n

This position primarily focuses on children from birth to three years old who experience developmental delays or disabilities. Our Children’s Services program emphasizes early intervention, providing much-needed support to infants and toddlers to help them achieve key developmental milestones in physical, cognitive, social/emotional, and communication skills.

Qualifications\n

• A Master’s degree or Doctorate in Social Work is required.
\n
• Current licensure as an LCSW through the Department of Public Health.
\n
• Must possess a valid driver’s license for potential travel to assist families.
\n
• A minimum of one year of experience in early intervention services or working directly with children is essential.
\n
• Demonstrated commitment to Natural Learning Environment Practices.
\n
• Ability to adapt to various learning styles to meet the individualized needs of each child.
\n
• Strong organizational, time management, and communication skills.
\n

Preferred Experience\n

Compensation and Benefits\n
