Key Responsibilities\n

• Conduct comprehensive assessments of clients to identify their mental health needs.

• Develop, implement, and monitor individual, person-centered treatment plans that adhere to a recovery-based model.

• Facilitate therapeutic sessions via telehealth, employing evidence-based techniques to foster coping, problem-solving, and communication skills.

• Engage in active case management, assisting clients in connecting to community resources and advocating on their behalf.

• Provide ongoing evaluation and adjustment of treatment plans based on clients’ progress and changing needs.

• Introduce and guide clients through skill-building activities aimed at enhancing daily living and community integration.

Patient Population / Client Focus\n

In this role, you will primarily serve adults aged 18 and older who are experiencing various mental health challenges. Many of these individuals may struggle with maintaining independence and stability within their communities due to conditions such as anxiety, depression, trauma, and other mental health disorders. Your focus will be on creating a supportive, understanding environment to empower them in their recovery journey.

Qualifications\n

• A Master's degree or higher in Social Work, Psychology, Counseling, or a related field.

• Current and valid licensure in the state of Illinois as a Licensed Clinical Professional Counselor (LCPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Clinical Social Worker (LCSW).

• Demonstrated ability to practice independently without supervision.
\n
• No disciplinary actions against your license.
\n
• Proficient in using telehealth technology and maintaining confidentiality.

• Excellent interpersonal skills with a focus on empathy and support.

Preferred Experience\n

• Prior experience providing telehealth services is highly desirable.

• Experience in community outreach and resource referral processes.

• Familiarity with symptom assessment and managing behavioral health issues.
