Work Environment\n

\n Working hours are Monday to Friday, from 9:00 AM to 5:00 PM, enabling a structured yet flexible approach to your commitment to care.

Key Responsibilities\n

• Act as a liaison between clients, families, and outpatient providers, ensuring clear communication and comprehensive support throughout the hospice care process.
\n
• Engage with insurance companies for pre-certification and utilization management, streamlining the administrative aspects of care that impact patient access to services.
\n
• Play an active role in discharge and aftercare planning by attending interdisciplinary team meetings and collaborating on personalized care plans.
\n
• Provide therapeutic meal support and educate clients on maintaining a healthy relationship with food, contributing to their overall wellness and psychosocial health.
\n
• Deliver safe, empathetic, and affirming care tailored to each client's unique circumstances, with an awareness of their intersecting identities.

Patient Population / Client Focus\n

The primary focus of this role involves working with older adults and individuals facing life-limiting illnesses. You will provide support in navigating the emotional complexities surrounding end-of-life care, allowing clients and families to embrace their circumstances with dignity and understanding.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Possession of a valid LCSW license in the state of California.

• Experience working with older adults or individuals dealing with critical health challenges.

Compensation and Benefits\n
