Work Environment

This position is entirely onsite, as our Physical Therapists work directly within the homes of patients. Key Responsibilities

• Conduct thorough assessments and develop individualized treatment plans that align with patients' functional goals.
• Implement therapeutic exercises and interventions designed to enhance physical function and mobility.
• Utilize state-of-the-art equipment and techniques, including modalities for pain management and neurological rehabilitation.
• Monitor patients' progress and adapt treatment plans as necessary, ensuring the highest quality of care.
• Educate patients and their families on exercise techniques, injury prevention, and lifestyle modifications.
• Maintain accurate and timely documentation of patient care activities, adhering to regulatory and compliance standards.

Patient Population / Client Focus

The primary focus of this role is working with adults who require physical therapy for various conditions, including post-operative rehabilitation, chronic pain management, stroke recovery, and mobility impairments. You will be instrumental in helping patients regain independence and improve their daily functioning within the comfort of their homes.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Valid Physical Therapy license in the state of California.
• A minimum of 2 years of experience in a relevant clinical setting.

Preferred Experience

While the minimum experience required is 2 years, we prefer candidates who have extensive knowledge of home health care practices and therapeutic interventions. Experience with geriatric populations or exposure to a diverse range of patient conditions will be highly regarded.

Compensation and Benefits
