This position is predominantly remote, allowing you the flexibility to create your own schedule and work from a location of your choice while serving patients at our hospice center through teletherapy sessions. Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of patients and their families to identify individual needs and develop tailored care plans.

• Engage in effective collaboration with interdisciplinary teams, participating in patient rounds and ensuring seamless communication regarding patient care and treatment interventions.

• Provide professional counseling services utilizing graduate-level clinical social work or counseling interventions, particularly focusing on Cognitive Behavioral Therapy (CBT) techniques and other evidence-based modalities.

• Manage and document patient care promptly and accurately, ensuring that all information is completed ethically and thoroughly, as per departmental policies.

• Maintain an active case management role for assigned patients by coordinating with hospital and community resources to deliver comprehensive support.

• Develop individualized treatment goals and collaborate with clients to promote participation and progress towards their mental health objectives.

Patient Population / Client Focus\n

The patient population you will serve includes individuals, adolescents, and families facing mental health challenges at our hospice center. Our diverse clientele may experience a range of issues such as anxiety, depression, grief, trauma, and adjustment disorders. Qualifications\n

• A valid and current license as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or a Doctor of Psychology (PsyD).

• A minimum of 2 years of clinical experience in a relevant mental health setting.

Compensation and Benefits\n
