Work Environment

This position is onsite with a home care focus, providing services in various private residences across the Middletown area. You will interact closely with patients in a compassionate setting, equipped with advanced therapeutic tools and resources. Key Responsibilities

• Provide exceptional patient care by performing thorough evaluations and developing personalized treatment plans based on individual needs and recovery goals.
• Adhere to all regulations provided by the New Jersey State Practice Act and ensure compliance with ethical standards set forth by the American Physical Therapy Association (APTA).
• Utilize evidence-based methodologies to deliver goal-directed treatments that optimize recovery, including manual therapy techniques, therapeutic exercises, and education on injury prevention.
• Continuously monitor patient progress and adjust treatment plans as necessary to promote optimal rehabilitation outcomes.
• Collaborate with other healthcare professionals, including occupational therapists, nurses, and physician assistants, to ensure a holistic approach to patient care.
• Document all patient interactions, evaluations, and progress notes accurately, prioritizing confidentiality and integrity at every step.
• Educate patients and their families about exercises, strategies, and resources available to enhance recovery outside of therapy sessions.

Patient Population / Client Focus

You will work with a diverse patient population, spanning pediatric, adolescent, adult, and geriatric patients facing various orthopedic and neurological conditions. Your ability to assess and engage with patients across this spectrum is essential in creating effective, age-appropriate treatment plans that cater to their specific rehabilitation needs.

Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited program.
• Current licensure in the State of New Jersey as a Physical Therapist.
• A minimum of 2 years of proven experience providing quality rehabilitation services.
• Strong understanding and familiarity with orthopedic rehabilitation practices.
• Excellent communication skills, with the ability to clarify complex concepts and empathize with patients from diverse backgrounds.

Preferred Experience

While not mandatory, candidates with experience in home health care settings or specialized rehabilitation programs will be given preference. Familiarity with outcome measures and data-driven practice is also advantageous for improving patient care strategies.
