Key Responsibilities\n

• Provide individual and family counseling, therapy, and psychotherapy, developing comprehensive treatment plans tailored to the unique needs of each patient.

• Conduct thorough assessments through interviews and case histories to gather insights into each patient's emotional and psychological state, ensuring a holistic understanding of their circumstances.

• Utilize observational techniques and data evaluation skills to identify underlying issues and recommend appropriate therapeutic interventions or referrals to specialists when necessary.

• Develop and execute discharge plans, ensuring smooth transitions and continuity of care for patients being discharged from hospice services.
\n
• Maintain a balanced caseload, encompassing both individual therapy and collateral sessions, ensuring timely follow-up and providing high-quality support to all clients.

• Regularly review client case files for accuracy, compliance, and timeliness, ensuring that all documentation, including care plans, assessments, and progress notes, fulfill clinical standards and regulatory requirements.

• Stay informed on best practices and health plan regulations, adapting assessment protocols as necessary to remain aligned with industry standards.

This role primarily focuses on patients who are navigating the emotional and psychological complexities associated with life-limiting illnesses. Our clients include individuals and families dealing with terminal diagnoses, requiring sensitive communication and specialized support. Qualifications\n

• Master's Degree in Social Work from an accredited institution, with a strong foundation in clinical practices.

• An active and unrestricted Clinical Independent Social Worker License (LICSW) is mandatory.

• A minimum of two years of relevant clinical experience in a healthcare setting, with at least one year in a mental health-focused environment.

Compensation and Benefits\n

The annual salary for this role ranges from $100,000 to $105,000, commensurate with experience and qualifications.
