Work Environment\n

This role offers the flexibility of a remote work setting, enabling you to connect with clients from the comfort of your home while serving a pivotal function within a hospice center. You will collaborate virtually with an interdisciplinary team of healthcare professionals, caregivers, and family members to ensure comprehensive support for our clients. Your schedule will generally be Monday to Friday, from 8:00 AM to 4:30 PM, with potential for occasional evening or weekend hours based on client needs.

Key Responsibilities\n

As a Licensed Master Social Worker at Bright Path Social Work, your responsibilities will include:

• Conducting comprehensive psychosocial assessments to evaluate the emotional, mental, and social needs of seniors tailored to hospice care.

• Developing individualized care plans in partnership with clients and their families, ensuring that interventions are aligned with clients' goals and preferences.

• Providing ongoing counseling and emotional support to clients and their loved ones, assisting them through transitions such as end-of-life care.

• Advocating for clients' rights and access to essential community resources, thereby facilitating their engagement with available support services.

• Engaging in frequent, scheduled visits to monitor clients’ emotional health and living conditions, ensuring their safety and comfort at home.

• Effectively communicating with families to resolve conflicts related to care and to foster understanding among all involved parties.

• Educating clients and their families about available resources, support groups, and programs that enhance their social connections and quality of life.

• Maintaining meticulous documentation of client assessments, care plans, and interactions, utilizing electronic health record (EHR) systems.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution is required.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW) is essential.

• A minimum of two years of experience in a social work role, preferably in a geriatric or community center setting.
